Card Info The Last Warrior from Another Planet
EARTH / Warrior-Type Fusion / Level 5 / ATK 2350 / DEF 2300
"Zombyra the Dark" + "Maryokutai"
When this card is Special Summoned, destroy all monsters on your side of the field except this card. As long as this card remains face-up on the field, neither player can Normal Summon, Flip Summon, or Special Summon any monster.
Comments: It'd be no doubt abused with Instant Fusion and probably have stayed banned with Instant Fusion still legal, especially considering its first effect would be activated.    
Card Info Dimensional Prison
Continuous Trap
During the Battle Phase, you can send 1 card you control to the Graveyard, except this card, to end the Battle Phase.
Comments: Not much of a "prison" when the effect doesn't imprison the monster at all is it? Though given that Scrubbed Raid isn't really used (and that's the text this card has), no one would've used it if this were how it was done.
Card Info Delinquent Duo
Normal Spell
Look at your opponent's hand. Select 1 card among them and return it to his/her Deck. The Deck is then shuffled.
Comments: Another case of two banned cards coming together in a sense because Delinquent Duo has The Forceful Sentry's text.   
Card Info Cyber-Stein
DARK / Machine / Level 2 / ATK 700 / DEF 500
Pay 1000 Life Points to Special Summon 1 level 6 or lower Fusion Monster from your Extra Deck in face-up Attack or Defense Position. That Fusion Monster cannot attack your opponent's Life Points directly, and is returned to your Extra Deck at the end of the turn.
Comments: Containing the effect of another banned Monster Card would ironically water down the effect given that it wouldn't be able to utilize its infamous OTK with Blue-Eyes Ultimate Dragon and whatever you bring out can't attack directly.
Card Info Goyo Guardian
EARTH / Warrior-Type Synchro / Level 6 / ATK 2800 / DEF 2000
Pay 5000 Life Points. Special Summon 1 Fusion Monster from your Extra Deck to the field in Attack Position.
Comments: No way to indicate how you Synchro Summon it and features the text of the notorious Level 2 monster that cost a pretty penny on eBay only to later get banned and knocked down to a Rare in DB2-EN.
Card Info Crush Card Virus
Normal Spell
Tribute 1 DARK monster with 1000 or less ATK. Check all monsters on your opponent's side of the field, your opponent's hand, and all cards they draw (until the end of your opponent's 3rd turn after this card's activation), and destroy all monsters with 1500 or more ATK.
Comments: It's sort-of taking a page from the dubbed version of the TV show where it was actually shown as a Spell Card. Seeing as it was already too powerful as a Trap Card,  it'd probably have followed Cyber-Stein with an emergency banning during the midst of the banned list currently in effect.
Card Info Premature Burial
Equip Trap
Pay 800 Life Points. Select 1 Monster Card from your Graveyard, Special Summon it on the field in face-up Attack Position, and equip it with this card. When this card is destroyed, the monster is also destroyed.
Comments: This'd have massive implications regarding how it worked because as it stands, Trap Cards that equip are Normal Trap Cards with effects allowing them to be equipped. This misprint can be taken as having the wrong template, or being the wrong card as a whole.
Card Info Red Dragon Archfiend
DARK / Dragon-Type Synchro / Level 8 / ATK 3000 / DEF 2200
1 Tuner + 1 or more non-Tuner monsters
If this card attacks a Defense Position monster your opponent controls, destroy all Defense Position monsters your opponent controls after damage calculation. During the End Phase, destroy all other monsters you control that did not declare an attack this turn.
Comments: Under this condition, you'd have the ability search it, though as an Extra Deck monster, it's right there. However, it's DEF is also incorrect, doubling the errors this misprint has.
Card Info Stardust Dragon
WIND / Dragon-Type Synchro / Level 8 / ATK 250 / DEF 2000
1 Tuner + 1 or more non-Tuner monsters
You can Tribute this card to negate the activation of a Spell Card, Trap Card, or Effect Monster's effect that destroys a card(s) on the field, and destroy that card. If you negate an effect this way, you can Special Summon this card from your Graveyard during the End Phase.
Comments: Not many Decks would consider it given its ATK is much lower than it actually is.
Card Info Call Of The Haunted
Normal Trap
Discard 1 card from your hand to the Graveyard to destroy 1 card on the field.
Comments: A pair of errors as this is a Normal Trap with a drastically different effect. Incidentially, it'd still have the same versatility though.
Card Info Monster Reborn
Normal Spell
Select 1 monster from your Graveyard and Special Summon it in face-up Attack Position. When this card is removed from the field, destroy the monster. If the monster is destroyed, destroy this card.
Comments: Well it still revives a monster from the Graveyard but with Call of the Haunted's text, it doesn't even make sense given that it's not a Continuous Spell and thus can't be destroyed. The way it's worded would mean it'd revive a monster, then destroy it, which I suppose has its uses but not very many.
Card Info Exodia the Forbidden One
LIGHT / Spellcaster / Level 3 / ATK 1000 / DEF 1000
An automatic victory can be declared by the player whose hand contains this card together with the Left Leg/Right Leg/Left Arm/Right Arm of the Forbidden One.
Comments: Being searchable with Photon Veil would probably result in PV being banned because that adds three more ways for Exodia to be searched from the Deck.
Card Info Makyura the Destructor
DARK / Warrior / Level 4 / ATK 1600 / DEF 1200
During the turn this card is sent to the Graveyard, the owner of this card can activate Trap Card(s) from his/her hand.
Comments: Doesn't really matter what colored border this guy has unless it were an Extra Deck Monster's border because it was never used to attack with, only dumped in the Graveyard to make use of the effect.
Card Info Torrential Tribute
Normal Trap
When your opponent Normal Summons or Flip Summons a monster with an ATK of 1000 points or more, destroy the monster.
Comments: It wouldn't be anywhere near as potent as it is today, though even back during LON days, it'd take on Trap Hole's reputation.
Card Info Dark Hole
Continuous Spell
Destroys all monsters on the field.
Comments: Well it'd be quite vulnerable but back then, there weren't many ways to destroy Spells or Traps so this'd sit there wiping the field endlessly.
Card Info Blue-Eyes Ultimate Dragon (original artwork)
LIGHT / Dragon-Type Fusion / Level 8 / ATK 4500 / DEF 3800
"Blue-Eyes White Dragon" + "Blue-Eyes White Dragon" + "Blue-Eyes White Dragon"
Comments: A few stars in its Level were shaved off. It's a pretty strong Level 8 Fusion Monster no doubt.
Card Info Mirror Force
Normal Trap
Activate only when your opponent declares an attack while they control 3 or more Attack Position monsters. Destroy all Attack Position monsters your opponent controls.
Comments: A more subtle misprint considering it has the text of Radiant Mirror Force, which has a more strict activation requirement.
Card Info Raigeki
Normal Spell
Increase your Life Points by 500 points.
Comments: Sadly, this could very well have been a believable foil card considering most foil cards in LOB were pretty bad. Of course, Raigeki wouldn't have become one of the most devastating Spell Cards released that made it banned for so long.
Card Info Gate Guardian
DARK / Warrior / Level 11 / ATK 3750 / DEF 3400
FLIP: Destroy 1 monster on the field.
Comments: Had this come to pass, Gate Guardian would've ironically actually had more uses.
Card Info Elemental Hero Flame Wingman
WIND / Warrior-Type Fusion / Level 6 / ATK 2100 / DEF 1200
"Elemental Hero Avian" + "Elemental Hero Burstinatrix"
This monster cannot be Special Summoned except by Fusion Summon. When this card destroys a monster and sends it to the Graveyard as a result of battle, inflict damage to your opponent's Life Points equal to the ATK of the destroyed monster.
Comments: I think you'll pick up the error on this one.
